If you experience difficulty accessing the portal or prefer not to use it, you may download the forms below, complete them in advance, and bring them with you to your first visit.

AFTER HOURS GUIDANCE

For after-hours needs, our answering service will assess your concern and contact the on-call provider if medically necessary. Please be aware that after-hours evaluations may include an additional fee. Non-urgent matters will be managed during normal clinic hours.
